This is one of the Forms needed for Spains Self Employment Visa – to be handed in for Phase 1 of the self employment visa application.

• Download the EX07 form.
• Present a complete and signed application form in duplicate to the consulate. Complete, print and sign 2 of these.
• Hand in with your other Phase 1 documents.

Modelo 790 Form for Payment

This Form is needed for Spains Self Employment Visa. Hand in for Phase 1 of the self employment visa application.

• Form for the fees. (Modelo 790.52) make 2 copies. Remember to get your Receipt.
• The payment receipt is important for tracking.
• This is the fee you pay on the day of your visit at the consulate in cash. It is about R 3800 (180 Euro) per applicant/ application.
